PARENTAL CONSENT
Program: Coach Hikmet Aslan Winter Indoor Futsal Program at Renbrook School, West Hartford, Connecticut (ages 6–14).
1. Consent to participate
I confirm that I am the child’s parent or legal guardian and authorize the child to participate. Activities include running, ball handling, agility exercises, drills, and soccer-related games. I will inform the organizer of relevant restrictions or accommodations.
2. Acknowledgment of risks
I understand that sports carry inherent risks, including falls, collisions, sprains, strains, and other injuries. Reasonable safety precautions cannot eliminate all risk.
3. Liability acknowledgment
I acknowledge the inherent risks of participation. To the extent permitted by applicable law, I agree not to hold Coach Hikmet Aslan or the program’s coaches and authorized personnel responsible for injuries arising solely from ordinary inherent risks of participation. This provision does not attempt to waive rights or claims that cannot legally be waived, including claims arising from gross negligence, recklessness, or intentional misconduct. The enforceability of any waiver relating to a minor requires Connecticut legal review.
4. Emergency medical authorization
In the event of injury or emergency, I authorize program personnel to contact emergency services and take reasonable steps to obtain medical assistance. Reasonable efforts will be made to contact me or the emergency contact provided. If I cannot be reached, emergency professionals may provide care as permitted by applicable law. I understand that I may be responsible for resulting medical expenses, except as otherwise required by law.
5. Parent and guardian responsibilities
I will provide accurate registration and emergency contact details; communicate relevant allergies, limitations, or accommodation needs; and encourage my child to follow reasonable safety directions and treat others respectfully.
6. Refunds and cancellation
I have reviewed the Program Terms and Refund & Cancellation Policy.
7. Electronic acknowledgment
By checking the required acknowledgment on the registration form, I confirm that I am the parent or legal guardian, have reviewed this agreement, and authorize the child’s participation and the emergency procedures described above. The consent is recorded with the registration and its agreement version.
Separate photo and video decision
This required acknowledgment does not authorize photography or video use. That permission is requested separately and is not a condition of participation.
